Please make sure to fill in and sign the first section of the form. Here is the link to the form: Preference Enrolment Certificate
If you are applying under 5.1, 5.2 or 5.3 we will need a copy of:
5.1 - The child's baptism certificate
5.2 - The sibling's baptism certificate
5.3 - The parent's baptism certificate
If you are applying under 5.4 an extra form will need to be filled out. Please follow this flow chart and make sure section A of this Supporting Evidence Form is filled in.
